Jerry Tam is opening his doors and letting all you lucky FORM friends in for a tasting of their exclusive goods. Cocktails will be served at this one night event.  Form’s events are totally worth attending. At Jerry’s birthday last Saturday Andre J and Sarah Roeme (the hot chick from Disturbia) were there to celebrate. Who knows who will show up on Wednesday.
